Licensed Substance Use Disorder/Addiction Coordinator LCSW/LPCC
and LCDAC Are you an experienced and compassionate Licensed
Addiction Therapist with a passion for guiding individuals through
their recovery journey? Signet Health is pleased to be partnering
with CHI Saint Joseph Health-Continuing Care Hospital in Lexington,
KY. We are seeking a seasoned professional to develop a NEW
substance use disorder program at our client’s (LTACH) Long Term
Acute Care Hospital. In this role, you will play a pivotal part in
supporting patients as they work towards lasting recovery and an
improved quality of life. If you have a deep understanding of
addiction therapy and thrive in this type of environment, this
opportunity is for you. Budgeted salary range of $95K based on
experience level. Responsibilities: Conduct diagnostic evaluations
and assessments to determine patients’ mental health and substance
use needs. Develop and implement effective personalized treatment
plans tailored to each clients unique circumstances. Maintain
accurate documentation of patient progress, treatment
interventions, and outcomes. Facilitate group therapy sessions and
educational workshops focused on substance abuse prevention and
recovery strategies. Advocate for clients needs within the
healthcare system and connect them with additional resources as
necessary. Excellent documentation and computer skills required.
Regularly review and assess patient progress towards their
treatment goals. Accurately and promptly enter all relevant
treatment information into the electronic health record.
Requirements/Qualifications Specific Qualifications: Master’s
degree with current KY licensure as a fully licensed clinician
(LCSW, LPCC) along with licensure in clinical alcohol and drug
specialty required (LCDAC). Minimum of 2 years of clinical
experience working with clients with substance use disorders (SUD)
required. Previous hospital-based inpatient or intensive outpatient
experience highly desired. Excellent communication skills, both
verbal and written, with the ability to build rapport with clients
