Job Description Job Description Job Type: Full-Time Job Summary: This resource coordination and connection position will focus on connecting mothers in the perinatal and/or postpartum period to needed mental health resources. In addition, this position will support these clients on connection to resources in the community that help to improve their quality of life and access to care. This position will require collaboration and coordination with a multidisciplinary team that could include therapist, medical providers, and other community partners. Success in this role requires cultural sensitivity, adaptability, and the ability to deliver services responsively to diverse families. Strong interpersonal and communication skills are essential for building trust and fostering effective working relationships and coordination across systems of care. Requirements: Must have a Bachelor’s degree in Social Work, Counseling, or a related field from an accredited institution Minimum of one (1) year clinical experience working with mothers or families Proficiency with Microsoft Windows and Office applications Strong documentation and time management skills Ability to work independently and collaboratively Key Responsibilities: Develop and coordinate Individualized Service Plans (ISPs) in collaboration with clients, families, and service providers. Facilitate team meetings and coordinate treatment with other providers. Assist clients in accessing community resources and support systems. Prepare meeting agendas, take notes, and ensure timely documentation. Adhere to agency standards and participate in professional development. Provide interventions based on best practices and engage in process improvement activities. Compensation & Benefits: $20.00 an hour with a $5,000 sign on bonus!
