As a Community Health Worker with Evolent's Medicare Advantage Dual Special Needs Plan, you will have the opportunity to make a profound impact on the lives of people living with multiple chronic illnesses as well as a vulnerable population with complex medical, behavioral and substance abuse issues. You will serve as a critical member of the integrated care team. . What You'll Be Doing: - Works under the guidance of an RN Care Advisor and/or Health Educator - Engage patients and care givers telephonically and in person in active care planning to focus on behavioral, clinical, social and environmental concerns and needs of patients. - Create and maintain a comprehensive inventory of local community resources, improving accessibility for patients and providers, and linking patients with the appropriate support services. - Assesses the patient's knowledge of their medical condition and provides education and self- management support based on the patient's preferences. - Connect patients with primary care, behavioral health, respite and other services. - Establishes relationships with community agencies, resources and supports that are relevant to SNP and Medicare Population. - Engage with patients who need assistance with self-care needs beyond what a nurse care advisor and/or health educator can provide via phone and/or face to face visits Address language and cultural barriers to care and self-care - Coach and guide the patient to meet both personal and clinical goals - Scheduling provider visits on behalf of their patients - Accompanying patients to their visits - Reminding patients of their upcoming visits - Helping patient access community and governmental based service agencies including possibly filling out paper work for the patient - Helping teach the care giver about symptom response plans - Arranging transportation - Facilitating closing gaps in care by educating patients about preventive monitoring and working with physician practices to schedule diagnostic testing - Participates in the integrated care team meetings Qualifications: The Experience You'll Need (Required): - Experience working with Medicare, Medicaid and Special Needs populations - Medical Assistant, Engagement Specialist or Community Health Worker Certification - Ability to travel throughout desired counties in Kentucky - Active, unrestricted driver's license Finishing Touches (Preferred): - Experience working with patients with chronic and behavioral health needs - Must demonstrate excellent interpersonal communication skills, the ability to engage with varying patient populations, and meet the patient 'where they are'. - Must be flexible and adaptable to change. Demonstrate the ability to work independently, as well as collaborate with multidisciplinary teams and health care providers.
