Job DescriptionThis job description outlines the objective, primary responsibilities and/or requirements, and qualifications of the ABI Care Team Coordinator.Objective: To coordinate ABI Companion Care Team to provide Medicaid compliance with client and companion files with the ultimate goal of assisting the team with providing excellent patient centered care.Reports to: Executive Director of ABI/Owner Primary Responsibilities (including, but not limited to):

  • Under direct supervision of the Executive Director of ABI where appropriate, assist with recruiting of companions by posting on job sites, pulling applications, etc.
  • Monitoring reminders to make sure that all Medicaid requirements are followed in regards to companion files (this includes, background checks, CEUs, TB, DL, Insurance, Performance Reviews and Supervisory Visits, etc.)
  • Oversee client paper and ClearCare client files to ensure that all client files meet the Medicaid requirements (this includes PA, POCs, Crisis Prevention Plans, etc.)
  • Manage Incident Report Binder to make sure that all incidents are documented and followed up on based on Medicaid requirements
  • Collect client Prior Authorizations from KyHealthNet website as needed
  • Operate as the office liaison between clients, caregivers and case managers as needed
  • Develop Menu of CEUs for companions with enough variety that we are not overlapping year to year
  • Develop and implement Companion Training program to meet Medicaid requirements to be used with all new hires in Louisville, Lexington and surrounding areas
  • Implement Red Cross/CPR training as needed in Louisville, Lexington and other areas as needed by companions.
  • Monitoring Onboarding Checklist for new client to include companion/respite hour review, notes entered in company software, Spenddown, Agreement for Spenddown signed, etc.
  • Monitor Respite usage weekly and report to ABI Team
  • Monitor Companion usage weekly and report to ABI Team
  • Answer the ABI Recovery Care phone
  • Monitor all client's schedules once a month in order to make sure that schedules are entered correctly and do not drop off due to not being extended out far enough
  • Minimize lost hours by assisting with finding companions to cover shifts and maximizing client schedule in relation to hours awarded.
  • Maximize profits by assisting with effectively matching pay rates with bill rates and by monitoring and minimizing companion overtime
  • Assist with scheduling as needed
  • Other responsibilities as assigned by the Executive Director and/or OwnerQualifications
    • Professional experience in the field of customer service
    • Knowledge of general healthcare staffing requirements
    • Team player, excellent verbal and communication skills, adaptable in different situations, possesses excellent client interaction skills, able to multi-task and work independently
    • Must have experience using telephones with multiple lines, multi-task on several calls and maintain professionalism and patience with a fast-paced environment
    • Excellent problem-solving abilities are a requirement, schedule conflicts are bound to arise that will require creative solutions
