PROGRAM: Determined Health - Indiana -
POSITION: Community Health Worker Tier 3
REPORTS TO: - Program Manager of Determined Health
STATUS: Full Time
OVERVIEW:
The community health worker will serve as a liaison/connector
between their client, the health care team, and the community.
Helps to bridge conversations with clients and remove barriers that
prevent them from accessing health and social services. -Community
Health Workers will primarily be working in the West end area with
specific target populations of partner insurance company identified
members. The CHW's will work closely with partner insurance
company, referral agencies, medical providers, primary care teams,
and other agencies to improve partner insurance company members
care and health outcomes. -
REQUIREMENTS:
1. High School diploma or equivalent required.
2. Two or more years of experience and/or a bachelor's degree.
3. Prefer candidates who live in southern Indiana.
4. Verifiable good driving record and reliable transportation.

5. Background check and fingerprinting. -
6. Openness to working closely with clients who struggle with
issues related to mental illness, substance use disorder, and
chronic health conditions. -
7. Respectful sensitivity and non-judgmental attitude towards
clients who may suffer from multiple traumas and/or
disabilities.
8. Strong interpersonal and social skills with an ability to
collaborate with a variety of individuals from a wide range of
professional and personal backgrounds.
9. Ability to convey a strong presence one-on-one, in meetings, and
in the community.
10. Ability to treat everyone equitably. -
RESPONSBILITIES:
1. Develops trusting relationship with clients by providing support
and advocacy to help achieve health goals.
2. Convey the purposes and services of Determined Health to the
member population and the impact that program or service.
3. Meets member in clinic, facility, office, or at home to help
identify social determinants of health impacting member's health
and general well-being.
4. Follow-up with health management/care plans with both members
and providers. -
5. Coach members in effective management of their chronic health
conditions and self-care.
6. Assist member in understanding care plans and instructions.

7. Document activities, service plans, and results in an effective
manner while strictly adhering to the policies and procedures in
place.
8. Conducts telephonic and/or face-to-face outreach to panel of
clients for appointment scheduling, needs assessment, and care gap
closure.
11. Help members in utilizing resources, including scheduling
appointments, and assisting with completion of applications for
programs for which they may be eligible. -
12. Facilitate communication and coordinate services between
providers. -
13. Identify and apply appropriate role definition and skilled
boundaries. -
14. Document all activities as detailed by direct supervisors.
15. Navigate clients to community resources to help address social
and economic needs. -
16. Approach each client utilizing a cultural humility and health
equity lens. -
17. Other duties as assigned.
